District Nursing Information

VISION OF DISTRICT NURSING:

"District Nurses provide quality, patient focused, professional, community nursing service."

MISSION:

"Delivery of  comprehensive nursing care supported by a range of speciality skills, with community and professional relationships, which enhances the ability of the individual and family to achieve their optimal health potential." (NZNO, District Nurses section)

DISTRICT NURSES:

  • Have a Primary Health Care focus and provide episodic care with links between primary, acute and specialist services to meet specific health needs in the community.
  • Use specialised nursing knowledge, assessment skills, professional judgement and knowledge of community resources and collaborate with other health professionals to
  • Provide care, advice, advocacy, health promotion, maintenance and restoration of health, preventative care, rehabilitation and palliative care (in the Wakefield, Tapawera and Motueka areas).  In the Nelson area Palliative care is carried  out by the Hospice staff.
  • Work in partnership with people regardful of cultures and are committed to the partnership, protection and participation of Te Tiriti o Waitangi.

District Nursing practice is evidence based and takes into account an holistic view of patients who vary in the complexity of their health situation.  It supports goal driven relationships that promote health improvement and independence, rehabilitation to optimal health and the professional care and support required by those in their care.

SERVICE PROVIDED:

  • Breast Care Follow up
  • Catheter Care
  • CAPD (Continuous Ambulatory Peritoneal Dialysis)
  • Congestive Heart Failure Follow up
  • Continence service. 
  • I.V. Therapy
  • Stomal Therapy
  • Wound Care
  • Oxygen therapy
  • Oncology and Palliative Care (Wakefield, Tapawera, Motueka District Nurses only)

Nelson:

  • Based at Nelson Hospital, Tipahi St, Nelson.
  • District Nurses provide a 7 day service.
  • Hours of work are 0800-1830.
  • Area includes Hira through to Three Brothers corner including City, Tahuna, Stoke and Richmond.
  • Clinic available by appointment.

Wakefield:

  • Based at Wakefield Community Health Centre, 12 Edward St, Wakefield.
  • District Nurses provide a 7 day service.
  • Hours of work are 0830-1500hrs Monday - Friday.  0800-1200 Sat/Sun.
  • Also provide Oncology and Hospice service so hours are variable as required.
  • Wakefield area includes from Three Brothers corner Richmond, Bronte Road Coastal Highway, Spooners Range Kohatu, and area within.
  • Clinic available by appointment

Tapawera:

  • This area is staffed on a Casual basis when the need is required.
  • The Tapawera nurse will at times work together with the Wakefield staff. 
  • Also provides Oncology and Hospice service.
  • Tapawera area includes Dovedale, part of the Motueka Valley (RD 2 Wakefield) and Lake Rotoiti.
  • Clinic available by appointment.

Motueka:

  • Based at Motueka Community Health, Courtney St, Motueka
  • District Nurses provide a 7 day service.
  • Hours of work are 0830-1630hrs.  As required Sat/Sun.
  • Also provide Oncology and Hospice services as required.
  • Motueka  covers Bronte Rd, Marahau, Upper Moutere and Baton Bridge area.
  • Clinic available by appointment.

REFERRALS:

  • All referrals are received via Health Professionals.  We do not accept self referrals but will assist you to access the appropriate service.
  • Referrals do not stay open on our books.  Once treatment is completed by the service patients are discharged.
