Chronic Conditions Coordinator
The Chronic Conditions Coordinator will be responsible for implementing a chronic care management framework in the Nelson Bays region, by consulting and collaborating with Nelson Bays general practices and Nelson Marlborough District Health Board. Close supportive and productive relationships with Maori providers and other key agencies will also be imperative to the success of this role.
