Maori Disease State Management Nurse covering Motueka and Golden Bay region providing clinics, home visits, or group Awhi support sessions. Available monthly in Golden Bay for individual or group appointments. Primarily works with Maori families. Physical activity
programmes and support groups available. A palliative care service is also available.
