Can do one-on-one or group consultations. Nutritional assessment and advice for weight management, type two diabetes, cardiovascular disease and other conditions for all ages. Individual assessment and education as well as Group sessions can be arranged. Community group presentations are also provided on request. Podiatry Services03 540 3758 Provides services to improve the podiatry care for individuals with diabetes and people at risk of diabetes. Te Amo Health03 528 5406 Maori Disease State Management Nurse covering Motueka and Golden Bay region providing clinics, home visits, or group Awhi support sessions. Available monthly in Golden Bay for individual or group appointments. Primarily works with Maori families. Physical activity programmes and support groups available. A palliative care service is also available. Te Hauora o Ngati Rarua Ltd03 539 0658 Maori Diabetes Nurse Educator to support people with Type 2 Diabetes providing clinics at Victory Community Health Centre, home visits, group Awhi support sessions. A Kaupapa Maori service of education and it has a holistic approach, focusing on the whole person, whanau/family, spiritual, mental, physical. Also a 6-week education course called 'Caring for Whanau with Type 2 Diabetes". Please phone for details. Also has a Tane Ora Coordinator which works towards improving Maori men's health
