How Green Prescription Works
Here's how the Green Prescription (GRx) process works for the health professional and patient.
- The health professional (e.g. General Practitioner or practice nurse) issues the patient with a Green Prescription (GRx script), provided the patient‚s medical condition is stable.
- The script is either written on a prescription pad (similar to a medication script) or issued electronically. It is then faxed through to the nearest GRx Office, which in our region is at Nelson Bays Primary Health.
- The script is picked up by our GRx Patient Support staff and contact is made by sending an information pack, followed by a phone call.
- The Patient Support staff member listens to the patient‚s likes and dislikes and together they develop an activity plan which encourages and supports the patient to become more active. Following the plan development, the patient receives:
- monthly telephone calls for 3-4 months, or
- a face to face meeting for extra motivation and encouragement
- The patient's progress towards an active lifestyle is reported back to the referring health professional.
- If the patient feels they would benefit from ongoing support, they are encouraged to ask their health professional for a repeat GRx.
