Falls Prevention Programme in Nelson Bays
Upright & Active Falls Prevention
ACC and Nelson Bays Primary Health have combined to develop this unique older adult wellness programme to the Nelson Bays region.
The programme is called Upright & Able Falls Prevention and it’s a 6-week programme delivered in small groups, which is FREE on the first referral.
- It incorporates education and samples a variety of suitable activity options such as Tai Chi, indoor walking circuits, Sit & Be Fit and home-based strength and balance options.
- It is held once a week at various locations around the region.
- Entry is for those who are 65+ years, who live independently, are at risk of a fall or have had a previous slip, trip or fall.
- A 'Falls Risk' is assessed on entry (and exit) to the programme. Anyone assessed as a ‘High Risk’ is referred onto NMDHB Physiotherapy Department for a more in-depth assessment.
- On completion of the 6-week programme on-going support is offered through our Green Prescription (GRx) programme to monitor and motivate activity habits.
The overall outcomes we hope to facilitate participants to achieve are: Older adults socially connected, confident, motivated and aware of falls risks. |

Education topics include:
- Preventing Falls in the Home
- Home-based balance and strength options
- Footwear and foot care
- Keeping bones strong with food
- Medications and Falls
- Goal setting and planning for the future

Activity options are dependent on what's available in that community but can include:
- Sit & Be Fit
- Tai Chi
- Home- based options
- Indoor Walking circuits
- Hydrotherapy
- Level 1 group programme in local gyms
