Care Plus Exit Advanced Form

From 1 January 2010, South Link Health will not process any Care Plus claims you send them.

In order to be able to exit a patient from Care Plus, you will need to use the NBPH Care Plus Exit advanced form to submit your exit form to Nelson Bays Primary Health.

Although the NBPH Care Plus Exit advanced form may look different to the old South Link Health form, you will use it in a similar way.

You only need to complete the Exit form if the patient is exiting the Care Plus scheme early - this is not required if you are re-registering the patient, or if they have used up all their reviews.

1. Press Shift-F3 to bring up the list of Advanced Forms, and select NBPH Care Plus Exit.

2. Fill in all the details on the Main tab of the advanced Form and click OK to complete the form.

The exit date defaults to today, and the form requires you to indicate that the patient's Care Plan goals were reviewed before you are able to submit it.

On completion of the form, the details are submitted to Nelson Bays Primary Health, who will update their database accordingly, freeing up a place on the Care Plus scheme for another of your patients.
